Checklist for new Families
To enrol your child you must ensure the following:
- You have thoroughly read and understood the OSHC Parent Handbook.
- Completed an online Application for Enrolment via one of the following links:
Evanston Campus OSHC
Two Wells Campus OSHC
- Completed Medical Authority Form or advise of any dietary plans, if applicable. Plans must include a start date and review date as advised by your medical practitioner.
- Contacted Centrelink if you wish to apply for Child Care Subsidy (13 61 50) as soon as possible as the process may take several weeks. Please phone the College at Two Wells on 8520 4123 or Evanston 0466 516 979 if you require any assistance.
